Destiny Moore, Chronic Disease Management Program

“I can honestly say I feel like two different people from then and now.” That’s how community leader Destiny Moore, 32, of Charlottesville describes the impact of Sentara Martha Jefferson Hospital’s free Chronic Disease management program in helping her fight diabetes. The program is made possible by support from community members like you.

Led by Sentara Martha Jefferson RN Setour Dillard, the program support Destiny as she learned new cooking techniques, overhauled her diet, and became a diehard label reader. Today, she is close to her weight goal. She no longer uses diabetes medication or her CPAP machine.

Destiny has far more energy to focus on raising her son and building the nonprofit organization, Growth Mindsets, she created to provide mentoring opportunities for young people. A UVA Equity Center Community Fellow, Destiny is committed to supporting children and families here at home in our community. We all benefit from her renewed vitality.
